Patient Import


How to import patients into the patient database

  1. Navigate to the Patient Import page
  2. Click the "Import Patients" button
  3. Select the file containing the patients you would like to import into the system.
    • .CSV and .XLSX file types are supported
  4. Click the Submit button to import the patient data displayed in the table

Import Template

The system uses a CSV template for patient information import.

The import template contains the following column headers:

Column header names for each data type must match exactly to be validated for import.

  • Between 1 - 10,000 patients can be imported per file import

File Format Corrections

The system validates data before creating patients in the database to ensure the data is properly formatted.

If an imported file is rejected, it is likely caused by one or more of the following reasons:

  • Missing data for first name, last name, or date of birth

  • Invalid date of birth format

  • Invalid phone number format


Pending release:

Duplicate patients

Duplicate patients are identified by the system during import to help preserve the integrity of patient database.

If a patient has identical first name, last name, and date of birth as another existing patient, this patient can be flagged for review to confirm it is not a duplicate.


Importing data from external parties

When working with external vendors, some field values should be provided to ensure patient data is properly associated.
